No. iPhones and most Android phones read QR codes straight from the camera app. People point the camera and tap the link that pops up.
About 2 × 2 cm is a sensible minimum for things people hold, like business cards and packaging. For posters and signs, work from the scanning distance: the code should be roughly a tenth of it, so 10 cm wide for someone a metre away.

It's the blank margin around the code. Keep it free of text and graphics, about four squares wide on every side, so phone cameras can find the corners.
